CORE SERVICES

Community Oriented Recovery and Empowerment Services

CORE Services are person-centered, recovery-oriented, supports intended to improve health by reducing stress, symptoms, and crises while building skills and self-confidence that promote and facilitate community participation and independence. 

CORE Services can help you to find and keep a job, go to school, live independently, build relationships, and increase your wellness. 

Eligibility

Adults 21 and over who are covered by Medicaid and enrolled in a Health and Recovery Plan (HARP) can get these free services with a recommendation from a Licensed Practitioner of the Healing Arts, such as physician, nurse, or mental health counselor. Additionally, some people enrolled in HIV-Special Needs Plans and Medicaid Advantage Plus Plans can also be eligible for CORE services.

  • Learn how to identify triggers, and manage or prevent crises 

  • Get help from people who have been there 

  • Learn from peers who know what you are going through

  • Help your family and friends learn skills that will support your recovery

Psychosocial Rehabilitation 

Assist the peer in improving their functional abilities to the greatest degree in the settings where they live, work, learn, and socialize through the use of rehabilitation counseling, skill building, and psychoeducational interventions. 

Parent and Child

Family Support and Training 

Offers instruction, emotional support, and skill building necessary to facilitate engagement and active participation of the family in the individual's recovery process. Conducted through the use of a person-centered, trauma-informed approach. 

Consultation

Empowerment Services - Peer Support

Offers non-clinical, peer-delivered services with a focus on rehabilitation, recovery, and resilience to promote coping with behavioral health symptoms while facilitating the utilization of natural supports and community resources.
