Lincoln School Apartment Program (LSAP)

  • Assists peers in obtaining safe and affordable housing

  • Provides access to effective treatment for optimal physical and mental/emotional health

  • Provides opportunities to participate in community roles

  • Helps peers build natural supports in the community

All referrals will be processed through the St. Lawrence County Single Point of Access (SPOA) Committee

OUR SERVICES

  • Furnished, kitchenette with appliances, and private bath with tub/shower

  • Includes electric, central heat and air, Wi-Fi access, and a laundry room on the main floor

  • Staff on-site 24/7 to ensure safety and provide support

  • Common areas to relax and socialize

ELIGIBILITY

  • Those who are unable to secure permanent and stable housing without assistance

  • Those who are inappropriately housed in an institutional facility and can safely live in the community 

  • Those who are at risk of homelessness due to their mental health diagnosis

REQUIREMENTS

  • Tenants are required to develop an Individualized Service Plan with a Housing Peer Specialist and work on goals and objectives to build skills, improve health, and secure income.

  • Tenants are required to follow the safety and security rules that apply to all tenants and staff at LSAP.

  • Tenants are required to respect others and the common areas as well as their apartments.

  • Tenants are required to actively participate in quarterly ISP reviews, take quarterly self-reporting surveys, have eligibility checked annually, review and discuss progress of their goals with their Housing Peer Specialist, and maintain income and benefits.
